All new patients booking requires you to place a credit card on file to hold any appointment, if you are in-network and using insurance it will be billed to your insurance and any copay will be charged day of your appointment. Before filling out all intake paper work it is required to go through a screening form first, once appointment is approved you will be sent all forms.
